I don't think you'll find any text-to-speech (or synthesised speech generally) to be perfect.
When I use the inbuilt text to speech I get a nice "lady" with an "English Accent"...it sounds acceptable to me.....but I supposed that's a matter of opinion.
There are other accessibility extensions (https://extensions.joomla.org/tags/accessibility/), you could try a couple of the free ones (there are not many) and see if they have any improvement, but to answer your question it doesn't seem like you can choose the voice in the default accessibility feature in J4/J5.
Perhaps you can post that idea for selecting the voice to Github as a "wishlist" feature (post ideas to https://issues.joomla.org).
